We offer case management for the Health & Wellness Waiver program. Our case managers seek to understand client's wishes and are committed to curating client-directed non-medical services and in-home care.
The Waiver has several requirements for ongoing eligibility to maintain services. Integrity Case Managers coordinates the in-home services & assessments required for waiver care management.
